Blower ran motor commonly in the $75-100 range if single-speed, $100-150 range if 2-4 speed, and $150-250 range if continuously variable speed. Labor for changeout probably about $100-200 range on most models if already opened up to replace the board. Of course, some brands run more just to be profiteers or because they are expensive imports.

Note if fan does not run after board is put in, more likely to be a fried airflow or firebox temperature sensor (sometimes several) that controls the blower than a bad motor, especially on a 15 year old unit - blower motors run for an awful long time unless exposed to a lot of incoming dust.
